1. Overview & Direct Evidence Gap

No controlled trial has evaluated whole-body hyperthermia (WBH) or far-infrared (FIR) sauna use as a treatment for a primary anxiety disorder (generalized anxiety disorder, panic disorder, social anxiety disorder, or related conditions).

Anxiety has been assessed only as a secondary outcome measure within trials built around other primary diagnoses, principally major depressive disorder (MDD) and fibromyalgia syndrome (FMS). This review states that absence plainly, reports the one directly relevant null finding, and distinguishes mechanism-transferred plausibility from anything resembling clinical evidence for an anxiety disorder specifically.

2. Anxiety as a Secondary Outcome: What Was Actually Found

Two studies tracked anxiety as a secondary measure in non-anxiety primary cohorts:

Mason et al. (2021)

A feasibility and acceptability study of a WBH protocol for major depressive disorder tracked anxiety symptoms using the GAD-7 alongside its primary depression outcome. The published result: the study did not find meaningful changes in measures assessing quality of life (WHO-5), anxiety symptoms (GAD-7), or positive affect (PANAS). This is a direct, specific null finding on anxiety symptoms in a WBH protocol that must be reported alongside any discussion of thermal mood effects.

UNRESOLVED MEASURE · GAD-7 IN FIBROMYALGIA (wIRA)

Langhorst et al. (2023)

A sham-controlled trial of mild water-filtered infrared-A (wIRA) WBH in fibromyalgia measured anxiety via GAD-7 in a subsample of 28 of its 41 participants (limited by "organizational problems" preventing full-sample measurement). Note: wIRA is a near-infrared band, not far infrared. No directional result for this GAD-7 subsample was reported sufficient to characterize here, and it remained an exploratory measure in a trial designed around pain.

3. Adjacent Mood Evidence — Not Anxiety-Specific

Two key trials address mood broadly in depression and are detailed in this series' companion Depression review:

Janssen et al. (2016) — JAMA Psychiatry

A sham-controlled trial finding WBH (delivered via near-infrared heating) produced significantly lower depression scores than sham in unmedicated adults with MDD. Neither enrolled primary anxiety diagnoses nor used anxiety as a primary endpoint.

Mason et al. (2025) — CBT + WBH Trial

A UCSF pilot combining WBH with CBT in which both active and sham-heat arms improved comparably, precluding a conclusion that heat added specific therapeutic benefit. Also restricted to depression cohorts.

4. Mechanistic Rationale — Transferred, Not Direct

Thermoregulatory and autonomic mechanisms proposed to underlie WBH's effect on depression (engagement of peripheral cooling responses linked to mood-relevant neural circuits) are sometimes extended by analogy to anxiety disorders, given shared autonomic nervous system involvement in both conditions.

Clinical Distinction: This is a plausibility argument, not a tested mechanism in anxiety populations. Crucially, the one anxiety-specific secondary outcome identified in clinical trials (Mason 2021) did not support this hypothesis.

⚠ Clinically Relevant Caution: Interoceptive Overlap

Panic disorder and related anxiety presentations frequently involve heightened interoceptive sensitivity to autonomic sensations — tachycardia, diaphoresis, and a subjective sense of intense heat — that overlap substantially with the physiological effects of deliberate heat exposure.

  • Not a Strict Contraindication: Controlled interoceptive exposure to similar bodily sensations is a recognized component of Cognitive Behavioral Therapy (CBT) for panic disorder.
  • Graded Introduction Required: Because physiological heating mimics panic onset, patient response cannot be assumed to be uniformly anxiolytic. A cautious, graded introduction is far more clinically appropriate than an assumption of default benefit.

6. Population Exclusions Observed in Related Trials

An ongoing trial of wIRA-WBH in post-COVID syndrome (NCT06204458) explicitly excludes patients with bipolar disorder, psychosis, schizophrenia, or personality disorder from enrollment.

This reflects standard trial-design caution around an untested intervention in these populations rather than a documented safety signal specific to WBH or FIR. However, because no trial identified has enrolled primary anxiety disorder patients, no population-specific safety or efficacy data exists beyond general clinical cautions.

Practitioner Summary: WBH and FIR sauna use should not be presented to patients or colleagues as a treatment for anxiety disorders. The only trial to measure anxiety as a defined secondary outcome in this context found no meaningful change. Where mood-related rationale is offered, it should be sourced to the depression literature explicitly and labeled as adjacent, non-transferable evidence.
