Far Infrared Therapy and Psoriasis / Psoriatic Arthritis

A Mechanistic Review with Explicit Separation from UVB Phototherapy

Overview

Psoriasis and psoriatic arthritis (PsA) present a specific communication risk for far infrared (FIR) therapy: patients are appropriately familiar with light-based treatment (UVB, PUVA) as an established psoriasis therapy, which creates a plausible but incorrect inference that FIR — also “light-based” in consumer framing — operates similarly or offers comparable benefit.

It does not. No controlled trial has evaluated FIR specifically as a treatment for psoriasis or PsA. This review separates the two modalities explicitly, presents the mechanism-transferred rationale for FIR at the appropriate evidentiary level, and flags the unverifiable claims practitioners are likely to encounter from patients.

The key distinction: Far infrared sauna use is not equivalent to UVB or PUVA phototherapy and should not be presented as a replacement for established psoriasis or PsA care.

FIR Is Mechanistically Distinct from UVB/PUVA Phototherapy

Ultraviolet phototherapy

UVB phototherapy (280–315 nm) is an evidence-based psoriasis treatment working through direct photochemical effects on epidermal keratinocyte proliferation and local immune modulation. PUVA combines UVA (315–400 nm) with a photosensitizing psoralen. Both operate in the ultraviolet band through photochemical mechanisms.

Far infrared

Far infrared (4–14 µm) occupies a different wavelength range and operates through thermal/vibrational tissue interaction, not the ultraviolet photochemical mechanisms described above. It does not replicate the keratinocyte-modulating mechanism that makes UVB effective, and should not be presented to patients as an alternative or equivalent light-based treatment.

Mechanistic Rationale for FIR

Non-specific to psoriasis and psoriatic arthritis.

NRF2/HO-1 and Inflammatory Signaling

FIR-driven HO-1 induction via the NRF2/ARE pathway restrains NF-κB-dependent inflammatory signaling (Lin 2008, ATVB), shown mechanism-specific and heat-independent in a controlled trial (Bagabir et al. 2025, Free Radic Biol Med).

Direct gene-expression data in an adjuvant-induced arthritis model demonstrate FIR downregulation of NF-κB, MAPK, and PI3K-Akt signaling (Chen et al. 2022, J Adv Res) — pathways centrally implicated in both psoriatic skin inflammation and psoriatic joint disease. None of this has been tested in a psoriasis or PsA population directly.

Adjacent Spondyloarthropathy Data

PsA belongs to the spondyloarthropathy family alongside ankylosing spondylitis (AS), sharing overlapping inflammatory pathophysiology and, in a subset of patients, HLA-B27 association.

A pilot study found infrared sauna produced statistically significant short-term improvement in pain and stiffness in AS patients with no exacerbation of disease activity ( Oosterveld et al. 2009, Clin Rheumatol, PMID 18685882 ). This is the most directly relevant adjacent-condition data available, though it was not conducted in PsA patients and should be represented as such.

Keep the population distinction clear: Findings in ankylosing spondylitis do not establish a treatment benefit for psoriatic arthritis.

Practitioner Advisory

Claims circulating in consumer content that could not be verified:

  • Vendor content reporting specific figures, such as “47% pain reduction” attributed to “the landmark 2009 study,” conflates and inflates the actual Oosterveld 2009 findings, which reported statistically significant but unquantified-by-percentage improvement, no PsA cohort, and did not measure IL-6/TNF-α percentage changes as claimed.
  • Testimonial claims of rapid, complete psoriasis clearance, such as “gone in less than a month,” circulating in patient forums and vendor content are anecdotal, uncontrolled, and should not be represented as expected outcomes.
  • No verifiable trial supports FIR as a direct psoriasis skin-clearing intervention. Practitioners should expect patients to arrive with these claims and should be prepared to redirect to the actual, more modest evidence base above.

Safety Considerations

Clinical Cautions

  • Heat may aggravate symptoms in some patients, while skin trauma can trigger new psoriasis lesions through the Koebner phenomenon. Response to heat exposure is patient-variable and should not be assumed beneficial.
  • Systemic PsA therapy — including TNF inhibitors, IL-17/IL-23 inhibitors, and methotrexate — warrants general infection-risk awareness in shared thermal environments, consistent with guidance for other autoimmune conditions in this series.
  • FIR must not be positioned as a substitute for phototherapy, topical, or systemic psoriasis/PsA treatment. It is, at most, a general supportive adjunct with no disease-modifying evidence.

Evidence Grading Summary

Domain Mechanism Evidence Human Evidence Tier
Skin clearance / plaque reduction None specific None — no controlled trials identified Absent
Inflammatory gene expression Strong (AIA model, direct) Not tested in psoriasis/PsA Mechanistic Only
Joint pain/stiffness (spondyloarthropathy) N/A Positive in AS (Oosterveld 2009); not PsA-specific Preliminary, Adjacent Population
Comparability to UVB/PUVA phototherapy None — distinct EM band and mechanism Not applicable Mechanism Mismatch

Clinical Positioning

FIR therapy has no established evidence base for psoriasis skin clearance and must be clearly distinguished from UVB/PUVA phototherapy in any patient conversation.

For PsA joint symptoms specifically, the AS pilot data offer modest, honestly labeled adjacent-condition support for general symptom relief alongside — never in place of — standard dermatologic and rheumatologic management.

Practitioners should be prepared to correct inflated pain-reduction and skin-clearance claims patients may bring from consumer or testimonial sources.

Supportive use does not establish skin clearance, disease modification, or equivalence to medical phototherapy.
